Standard Right to Know Law Request Form
This is Pennsylvania's Standard Right-to-Know Law Request Form, used to request public records from government agencies. Any legal resident of the United States may use this form.
- Agency must respond within 5 business days (may be extended 30 days)
- You have 15 business days to appeal after a request is denied or deemed denied
- Requests may incur fees; the agency will notify you if fees exceed $100 or an amount you specify
- Provide detailed description of records sought, including subject matter, timeframe, and type of record
- Indicate preferred method of receiving records: printed copies, electronic, or in-person inspection
