Standard Right to Know Law Request Form
A standardized Pennsylvania Right-to-Know Law (RTKL) request form for submitting formal requests for public records to government agencies. Available to legal residents of the United States.
- 15 business days to appeal if request is denied or deemed denied
- Agency must respond within 5 business days of receipt (or up to 30 days if extended)
- Requests may incur fees; you will be notified before processing if fees exceed your specified threshold
- Must affirm that your full name and contact information are true, correct, and that you are a legal resident of the United States
- Records available in printed copies, electronic format, or in-person inspection
- Submit to the agency's designated representative (Attn: AORO) via email, U.S. mail, fax, or in person
